Add-Drop, Change to Pass-Fail, Cross-Registration, and Other Forms

In the case of adding/dropping a course or changing it to or from pass-fail, you need the signatures of your concentration advisor and your instructor (you don’t need the instructor’s signature if you are only dropping a course). Cross-registration forms require that you get the signature of the instructor AND the Registrar for the host school, before getting my signature.

Adding a Course - instructor signature, concentration adviser signature, then me

Dropping a Course - concentration adviser signature, then me

Pass-Fail/Grade - instructor signature, concentration adviser signature, then me

Cross Registration - instructor signature, other school’s registrar, then me
